BG151616451 Ihad the same issue and wound up breaking one of the inserts when I tried to put it back in. I didn't have the proper thread insert installer so this turned into an opportunity to buy more tools. Also, one of the brass screws for the clamping bar was about 2mm longer than the other and protruded far enough to interfere with insertion in the rail even after fixing the thread insert issue. Had to grind it down, but now the rail square should work great.

QC and assembly issues can really mess up what should be a great tool. Customer support wasn't really any help either.

BG151616451 Ihad nearly identical issues. One of the helicoil inserts was installed such that the locking bar could not be inserted at all, and the other protruded just enough to leave scratches in the rail. Apparently I'm no longer a purchaser and can't leave a review though I was only refunded shipping charges. I wound up trying to fix it myself but the tab on one of the inserts broke while reinstalling. Wound up having to order some and picked up the proper installation tool to fix it properly. Also had to grind a couple mm from one of the brass knobs as it protruded well past the end of the locking bar.

Seems like a great tool, marred by poor QC in assembly.
